Application for Alternate Site Transportation

This form must be received to be eligible for alternate transportation (afterschool program, daycare, grandparents, etc.) for the school year. 72 hour notice for any change of transportation must be given to the District’s bus services. NO EXCEPTIONS.

Alternate site transportation must be five days per week and is not for requesting transportation to multiple locations throughout the week.

The requested alternate site must be located within the Troy City School District.

This form ensures the student transportation record has correct pick-up and drop-off locations.

This form is for public and non-public schools.
