What you need:
- A yellow sports approval packet needs to be filled out and returned to the health office. Sports approval packets are located below. You can fax, scan or mail the completed form back to the health office (TMS/THS).
FOR TROY MIDDLE SCHOOL STUDENTS:
Sports approval forms for TMS students need to be dated and turned in within 30 days from the start of the sport
FOR TROY HIGH SCHOOL STUDENTS:
Sport approval forms for THS students need to be completed and turned in as soon as possible (updates available within 30 days.)
