Auditorium Reservation

If you are an outside organization

Step 1 – Request the dates and times by electronically submitting a District Auditorium Use Form to Lori McAllister.

Step 2 – If your date and time are approved your District Auditorium Use Form will be emailed back to you with the approved signature.  At that time all information will be placed on the Auditorium Calendar and Technology, Custodial Staff, Maintenance and the TMS Executive Principal;  TMS Executive Secretary will be notified of the approved request.

* If your date and/or time are not approved, you will be contacted by Lori McAllister and a new date and/or time will be explored.  When a new date and/or time has been agreed upon, all information will be placed on the Auditorium Calendar and Technology, Custodial Staff, Maintenance and the TMS Executive Principal; TMS Executive Secretary will be notified of the approved request.

Step 3 – Submit a Use of Facilities Form and District Auditorium Use Form electronically, to the Troy Middle School Executive Principal.  Upon approval, an email will be sent to you stating that your Facilities Request Form has been approved and the form will be sent to the Business Office.

Step 4 – When step 3 has been completed; submit a Proof of Insurance Form to the Business Office.  Proof of Insurance information is mailed to: Adam Hotaling, Assistant Superintendent for Business, 475 First Street, Troy, NY 12180

Step 5 – Your request to use the District Auditorium at Troy Middle School is finalized when you receive an email from the Business Office stating that your Proof of Insurance, Request for Facilities Use, and Auditorium Use Form have been submitted to the Business Office.

 

If you are a Troy CSD employee

Step 1 – Request the dates and times by electronically submitting a District Auditorium Use Form to Lori McAllister.

Step 2 – If your date and time are approved your District Auditorium Use Form will be emailed back to you with the approved signature.  At the time all information will be placed on the Auditorium Calendar and Technology, custodial staff, maintenance and the executive secretary will be notified of the approved reservation.

* If your date and/or time is not approved, you will be contacted by Lori McAllister and a new date and/or time will be explored.  When a new date and/or time has been agreed upon, all information will be placed on the Auditorium calendar and Technology, custodial staff, maintenance and the executive secretary will be notified of the approved reservation.
